All parents and guardians are asked to serve one Sunday each quarter. If you are serving in another ministry you are exempt.
Households with two guardians will serve 2x
Households with one guardian will serve 1x
The following contact has permission to pick up your child from class.
This will also be an additional contact if we cannot get ahold of primary caregivers in case of emergency.
Please do not use a parent as an emergency contact. We require a third contact if parents cannot be reached during an emergency.
We may occasionally take pictures of children in the Sunday School programs and use them for a variety of publications for Antioch Bible Church (ex. website, social media, bulletin).
May we use photos of your child in Antioch publishing?

In case of medical emergency, I understand that every effort will be made to contact the parent first and then the emergency contacts. In the event that none of the said persons can be reached I hereby give permission to notify a physician and give permission to that physician to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery for the individual registered. In consideration thereof, I agree to hold harmless Antioch Bible Church (ABC) and/or its agents or representatives from any and all claims for expenses incurred, if any. I further hereby release and hold ABC, their agents and assigns, harmless from any injury or loss I, the participant, may suffer arising out of my involvement with said organization, except for negligent acts which would have been prevented if prior actual knowledge and reasonable opportunity had existed wherein the negligence would have been avoided by ABC. Except for that limited scenario, I take complete responsibility for my own acts. I further understand that the Bible states that Christians should not sue their fellow Christian (I Cor. 6:1-8) and I agree to those terms.
